I have visited Dr surajballi in Phoenix 3weeks ago.i was happy with the Dr. However I was totally dissapointed with the response and assistance I have received from the reception.i had requested my diagnosis report on the 1st day of my visit.i had not received it.i had a prescription emailed to me with 2eye drops.but no orders as to when I should use it.i rhen used 1 in the mng n 1 at night.i then had severe vision impairment.i tried numerous x calling but no one answered nor did anyone return my call.i then had a flow up appointment. Which was not confirmed.i again tried calling with no assistance.i eventually had to go to the rooms personly to get my appointment. Amd then again I requested my report.i did not receive it.after my appointment.i was surpose to be given another prescription which I did not receive together with my report.yet again I called at reception and requested this.ni response gone 3days.i tried calling 19x on Monday which I spoke to the lady 2x but still did not receive it.iwhwn eventuly i sed the lady i wad gna askmy medical aid to stop payment.about 15 min after that i received it.i requested for this to ne sent to my dr.which was not even done.after such a long wait then I get it.it had no details besides of me having glycoma.it was not even a complete report. Dr surajballi I appreciate your service you really are very good.however I will not recommend anyone to visit your rooms due to the lack of professionalism received.if your staff is overworked you should consider another staff to assist with paperwork.VERY DISSATISFIED WITH SERVICES RECEIVED
