Reissuing of HIRA Certificate

Good day,

My name is *****gwe Salome Mokumo (Mokgolobotho).

I completed a HIRA (Hazard Identification and Risk Assessment) course with NOSA in 2015. Unfortunately, my house was burnt down and I lost my original certificate in the fire.

Since then, I have made numerous attempts to contact NOSA for assistance in obtaining a reprint, statement of results, or confirmation letter. I have sent several emails and even went to their offices in person. However, I have been sent from pillar to post without any resolution or proper assistance.

This certificate is very important for my employment opportunities, and I am extremely disappointed by the lack of support and response despite my repeated efforts.

I am respectfully requesting that NOSA assist me urgently with:

• A reprint of my HIRA certificate • A statement of results • Or an official confirmation letter of course completion

I hope that by raising this matter here, it will receive the attention and resolution it deserves.
