Incompetency

Have never had a good experience at this hospital. I am currently in need for my mom doctor to complete forms he is unable saying that documents are in achieve the doctor has done all he can from his side but the hospital doesn't seem to want to assist him in getting the information this is truly frustrating and is really on. Sitting for 2 weeks now and nothing and this forms are needed.
