Refuse to submit unemployment documents to employee

I was employed for 12 months at Bradian Logistics Solutions. When my contract expired I requested my UIF documents to apply for UIF but Bradian Logistics refuses to send me my UIF documents. My UIF application keeps being rejected as I don't have correct document to be approved. When I phone bradian Logistics HR is always is a meeting and my information on the UIF system is incorrect as Bradian Logistics never updated and uploaded my UIF contributions as UIF call centre agent states my last UIF contribution is MARCH 2024 which is incorrect and false as I only began commencement in SEPTEMBER 2024. As of today I am unemployed and still awaiting unemployment documents from Bradian Logistics Solutions.
