Dear Practice Manager,
I am writing to lodge a formal complaint regarding a serious breach of my personal information by a receptionist at Dr Minaar’s practice, Wilma. I called this morning and spoke to Claudette.
Wilma accessed my details from the practice files and disclosed them to telemarketers despite the fact that she does not know me personally and had no legitimate reason to access or share my information.
This is a clear violation of patient confidentiality, privacy laws, and professional standards. I am extremely concerned about how my personal information was handled and the lack of safeguards to prevent this type of misconduct.
I request:
A full investigation into this incident.
Written explanation from Wilma
Confirmation that my personal information is now secure and will not be misused again.
Please treat this matter with urgency and seriousness. I expect a formal response within a reasonable timeframe.
